Can Chiropractic really help my Chronic Back Pain? Here’s what the research says.

Although most people associate arthritis with older people, shoulder arthritis is relatively common among younger and middle aged adults, too. The fact is any injury to the shoulder, such as a shoulder dislocation, may lead to shoulder arthritis.

Within the shoulder there are two joints: the glenohumoral joint and the acromioclavicular joint. The former is the larger joint and is a ball and socket formation. It is formed by the top of the humerus fitting into a cavity in the shoulder blade (scapula). The acromioclavicular joint is smaller and forms where the scapula and the clavicle meet.

Cartilage located at the ends of these bones is responsible for the movement of the humerus in the socket with a minimum of friction between the two. When a loss of cartilage occurs in these joints the bones experience greater friction when rubbing against each other. Shoulder arthritis is less common than other types of arthritis but it is often very painful and even debilitating.

Those who have shoulder arthritis tend to experience a progressively intensifying pain in the shoulder joint, often even when the arm is not being used. Stiffness in the joint and sometimes clicking or cracking may be felt when these two uneven surfaces rub against one another. Occasionally the shoulder may become very stiff or even lock up.

The most common symptom of shoulder arthritis is gradually intensifying pain, particularly when the arm is moved. People with this condition are also liable to experience significant stiffness in the joint and weakening of the shoulder. Sleep may be disrupted more frequently as the condition deteriorates, especially on the worse of the two sides.

As far as causes go, look to any of the following:

    1. Osteoarthritis: Degenerative wearing away of cartilage, particularly at the acromioclavicular joint.
    2. Traumatic injury to the shoulder resulting in loss of cartilage, such as from an accident, especially if there has been a tear in the rotator cuff.
    3. Rheumatoid arthritis: Inflammatory autoimmune disease whereby the body attacks its own cartilage.

To begin with, management of shoulder arthritis is typically non-surgical. Some treatment options may include:

    1. Chiropractic adjustment
    2. Exercise programs which target specific areas to increase mobility in the shoulder
    3. Heat or ice applications
    4. Shoulder immobilization and rest
    5. Supplements such as glucosamine and chondroitin, which have been shown to build cartilage and slow degeneration
    6. Non-steroidal anti-inflammatory drugs (NSAIDs)
    7. In the case of rheumatoid arthritis medications such as corticosteroid injections and others may be helpful

 

The Arthritis Alliance of Canada recommends the involvement of chiropractors in the identification, referral and treatment of arthritis. Chiropractic treatment can help relieve pain, restore mobility and increase range of motion in the shoulder joints. In severe cases, surgery for shoulder arthritis may help to reduce pain and improve range of motion if non-surgical treatments are unsuccessful.
